The Strata Council may require proof of ownership for the above-noted animal at any time. The following are examples of acceptable proof of ownership: original purchase receipt, veterinarian bills or city licensing.

This form must be returned to the Strata Council within 14 days of receipt. Failure to comply may result in your animal(s) not being permitted in the building. Once completed, please attach a photograph of pet for reference purposes.
